Region I Youth Programs
Programs consist of Integrated Care Coordination Unit ("ICCU"), Reach Out Foster Care ("ROFC") and Professional Partner Program ("PPP")
1) The family is a unit and is the primary focus of attention
2) Emphasis is placed on assessing and building on family strengths and on the capacity of families to function effectively.
3) Families are engaged in designing all aspects of the treatment, planning and evaluation.
4) Families are linked with more comprehensive, diverse, and community-based network of supports and services.
Region I Youth Programs focus upon Family Centered Practice
Family Centered practice works with the family as a collective unit, insuring the safety and well-being of family members through developmental services, support, problem-solving, planning and continual implementation of team-focused strategies. The primary purpose is to strengthen the families potential for carrying out their team, chosen by the family for support and assistance in outcome measures. The teams consist of the coordinator (Professional Partner, Care Coordinator or Program Specialist), who partners with families to use their expert knowledge throughtout the decision and goal-making process to provide individualized, culturally responsive, and relevant services for each family. Included in the family centered practice model are an array of services, supports and interventions that assist in mobilizing resources to maximize communication, shared planning and collaboration among the community and/or neighborhood systems that are directly involved in the family.
